Join the CoOp and Get Contracted

  • STEP 1: Download the CoOp Membership Agreement and save it on your desktop or in a folder. The form is an interactive PDF and can be filled out electronically. You may also print it, fill it out by hand, and scan before submitting the form.
  • STEP 2: After downloading, completing, and signing the CoOp Membership Agreement, submit it to the CoOp by clicking the Submit button, and attach it to the bottom of the submission form. Then click the Return button to continue with Step Three.
  • STEP 3: Purchase your annual CoOp Membership here. Once you’ve completed your purchase, click the back arrow to return to the “How to Get Contracted” page and continue to Step 4.
  • STEP 4: Download the forms file and save it on your desktop or in a folder. The forms are interactive PDFs and can be filled out electronically. You may also print, fill them out by hand, and scan before submitting the forms. Please note: the W-9 must be completed, printed, and signed by hand before scanning to submit.
  • STEP 5: Once you’ve completed and signed the broker forms, your final step is to submit them by clicking the Submit button, and attaching them to the bottom of the submission form.
